プリオン病
プリオン病とは脳神経細胞に細胞毒性と感染性を持つ異常プリオン蛋白が沈着することで、脳神経細胞が障害されて起こる疾患の総称である。
孤発性Creutzfeldt-Jakob病(クロイツフェルト-ヤコブ病)
細胞膜表面に存在する正常プリオン蛋白が何らかしらの原因で異常プリオン蛋白に変化することによって発症する。
異常プリオン蛋白は周りの正常プリオン蛋白を異常プリオン蛋白に変えてしまうため、突然発生した異常プリオン蛋白は雪だるま式に増えていく。
正常プリオン蛋白は代謝されるため神経細胞に蓄積することはないのだが、構造が変化した異常プリオン蛋白は代謝されずに神経細胞に蓄積して細胞毒性を発揮してしまう。
孤発性Creutzfeldt-Jakob病は1年間で100万人に1人の割合で発症する原因不明の疾患であるが、プリオン病のほとんどが孤発性Creutzfeldt-Jakob病であり代表的なプリオン病という位置づけとなっている。
クールー病
死者を儀式的意味で食べるという食人風習を持つパプアニューギニアの原住民の間で広がったプリオン病を指す。
最初は自然発生した孤発性Creutzfeldt-Jakob病の死者を食べたことによって、食べた人たちが次々とプリオン病を発症して死亡して連鎖的に広がったと考えられている。
原始社会での儀式的共食いが引き起こした伝染性のプリオン病といえる。
牛海綿状脳症(BSE)
牛が発症するプリオン病である。脳組織が海綿状(スポンジ状)になることから名付けられた。
最初は孤発性に牛海綿状脳症を発症した牛を原料とした飼料を食べた牛が発症することによって、連鎖的に広がったと考えられている。
食肉を除いた後に残った骨や屑肉や脳や脊髄を原料として作った肉骨粉を飼料として他の牛に与えたことが原因となった。
近代畜産における強制的共食いが引き起こした伝染性の牛のプリオン病といえる。
日本でも発生したが、発生数はヨーロッパが圧倒的に多い。
日本ではELISA法やウエスタンブロット法によって異常プリオン蛋白を検出するスクリーニング検査を行うことで対応している。
変異型Creutzfeldt-Jakob病
牛海綿状脳症(BSE)を発症した牛を食べたことによって、牛で発生した異常プリオン蛋白が牛から人へと種を超えて感染して人が発症したプリオン病であると推測されている。
脳や脊髄に異常プリオン蛋白は蓄積するため、牛の食肉処理の際に脳と脊髄は除去・焼却するように現在では法令上義務付けられている。
医原性Creutzfeldt-Jakob病
移植感染
過去にはCreutzfeldt-Jakob病の死者由来の乾燥硬膜や角膜や成長ホルモン剤が医原性の感染を引き起こして問題となった。
器具感染
過去にはCreutzfeldt-Jakob病の患者に対して使用した脳外科手術器具などを別の患者に使用した際に医原性の感染を引き起こして問題となった。
異常プリオン蛋白は通常の消毒や加熱処理では感染性を失わない。
手術器具は可能な限りディスポーザブルなものを使用する。
捨てられないものは、ドデシル硫酸ナトリウム(SDS)煮沸によって異常プリオン蛋白を変性させるという処理が必要になる。
感染対策
脳脊髄液検査や脳脊髄液手術では異常プリオン蛋白に曝露されて医療従事者が感染する危険性もあるので、感染対策に注意する必要がある。
以上に挙げたように医原性Creutzfeldt-Jakob病の危険性があるとはいっても、実はプリオン病ではウイルスや細菌が感染源となるのではなく異常プリオン蛋白が感染源となるため、感染源に曝露される機会が少なく感染性は高くない。
例えば、Creutzfeldt-Jakob病は唾液や尿や便や入浴や性行為といった日常生活で感染することはない。
日常生活における感染の可能性は極めて低いため、Creutzfeldt-Jakob病の患者は一般病棟に入院することが可能である。
症状
ミオクローヌスと呼ばれる不随意運動を伴いながら月単位で進行する急速進行性の認知症を呈する。
最終的には無動無言となり意思疎通が不可能となって、寝たきり状態のまま死に至ることが多い。
ミオクローヌス
ピクつくような素早い不随意運動を指し、腕全体がビクッとするような動きがみられたりする。
検査所見
脳MRI拡散強調画像
大脳皮質に沿った高信号を認める。
脳波検査
周期性同期性発作波(PSD)と呼ばれる特徴的な脳波を認める。
脳脊髄液検査
脳脊髄液に14-3-3蛋白と呼ばれる特徴的な蛋白を検出する。
